Everything I need is in your history and getting to it properly takes an hour. Every woman with pain during sex begins here and you leave with a clear idea of what to do next.

What the sixty minutes covers

We start with your full history, the whole sequence, what hurt first and what changed it and what you were told each time, including the parts you have stopped bothering to mention because nobody followed them up. That means where the pain sits, at the entrance or on the vulva or deep inside, what happens during sex and afterwards, what happens with tampons, what the burning does when the swabs come back clear and what happened the time you tried dilators on your own.
Most of the answer is in the history. Where I need to see something rather than hear about it, I will ask you to show me how you breathe, where you hold tension and how you move, because those things tell me what the muscle is doing without anyone touching you.
At the end I will tell you what I think is causing the pain, in plain language, and where to start. It stands on its own, so it is worth doing whether or not you go any further.
What happens if you need a physical exam
Some things cannot be assessed on a screen and I will not pretend otherwise. If something in your history points to a problem that needs a physical examination, imaging or investigation, I will name it, tell you why I think it needs looking at and direct you to the right route, whether that is your GP, a gynaecology team or a pelvic health physiotherapist who can examine you.
A referral does not end our work. The tension is still there while you wait and waiting is usually the longest part of the whole process. The breathing, downtraining and graded work carries on through it, so by the time the appointment comes round you are further on than when you were referred.
